SIMToolkit tutorials and events?

Hi,

I want to programm a JavaCard with SIMToolkit. The only description or tutorial to the SIMToolkit (not JavaCard) I found is the official 3GPP specification 11.14

Is there any tutorial how to use the SIMToolkit with Java(Card)?

I found out that the application could be notified if an event happens on the mobile phone, such as CallStarted. But what events are available, is there a list of all events?
