Poor software GUI and usability design. Examples :

This may be slightly off topic, but as a GUI developer I come across software (that I am forced to use) that I wished would burn in the pits of hell, because it is so poorly designed. My productivity plummets because I waste a large percentage of my day figuring the software out.
