update manager Unexpected Failure: java.lang.Exception: Sequencer failed.

Hi, I have the next error with update manager:

Unexpected Failure: java.lang.Exception: Sequencer failed.

the same error apears when I try to use smpatch or "check Updates" in the gui.

The logs files look like this:

Hello,

The error which you are seeing is on which we have encountered before and may be a result of a a bad Veritas patch.

Please check to see whether you have patch 117499-02 installed:

# showrev -p | grep 117499-02
